Pyopericarditis is a rare surgical condition. Five patients were seen over a period of ten years. An aggressive surgical approach was mandatory in order to get continuous adequate drainage from the most dependent part of the pericardium. In all patients the aetiology was due to pyogenic bacterial invasion, probably due to immune suppression associated with previous exposure to pulmonary tuberculosis. Two patients died while the surviving three were further subjected to anterior pericardiectomy.
